Descriptionof Position:

Provide overall care and support to the residents of Maple Cottage Assisted Living. Promote resident wellbeing and satisfaction through direct personal care and support with activities of daily living. Complete housekeeping assignments and follow the lead and coaching of the Lead CarePartners.Thenurseonshiftisalwaysthechargenurse.CarePartnerswillfollowdirections given by the nurse on shift.

StaffingPattern:

Maple Cottage Assisted Living staffs through a primary care structure. Each Care Partner will be chargedwithall,ofthepersonalcaredutiesforeachresidentassigned.Wheneverpossibleeach Care Partner will be assigned the same resident group each day to promote continuity of care.

ResponsibilitiesoftheCarePartner:

1. Assistwithactivitiesofdailyliving,includingassistancewithresidentself-medication.
2. Follow licensing regulations, safety policies and direction at Maple Cottage, including properliftingtechniqueanduniversalprecautionswhenprovidingcaretotheresidents.
3. Beknowledgeableoftheresident’scareplanandrefertoitintheperformanceof personal care duties.
4. Monitorresidentactivity,foodintake,functionalstatus,andpsychosocialstatus.Take action, as required, to promote resident wellbeing.
5. Reportstatuschangesandfollowdirectiongivenbythenurseonshift.
6. Actimmediatelyonanyresidentcrisis,followingprotocolandpolicy.
7. Promoteopencommunicationbetweenhealthcareprofessionals,families,residents and staff.
8. CompleteCarePartnerassignmentsincludingbutnotlimitedto,cleaningandlaundry, making rounds, dining services, activities, and tasks as directed by supervision.
9. Functionasateam,assistingcoworkersastheneedarisesandfollowingLeadCare Partners.
10. Adheretoguidelinesintheemployee handbook.
11. Completetrainingassignments
12. Other duties as assigned
